A groundbreaking study has revealed that frequent heading in football may cause subtle yet measurable brain damage, even in amateur players who have never reported any direct head injury.
Led by Dr. Michael Lipton, a neuroscientist at Columbia University in the United States, the study found structural changes in the brain's frontal region; specifically in the cortical gray matter-white matter interface (ZWI), among those who head the ball over a thousand times a year.
ZWI is a critical region where two key brain tissues; gray matter and white matter, connect. Damage in this zone could potentially disrupt brain communication and functioning.
The study observed 352 amateur adult footballers, each with at least five years of playing experience and active participation in the sport within six months prior to the study.
Brain scans revealed that players with a higher frequency of heading exhibited distinct structural alterations in ZWI area. Notably, these changes were consistent across age and gender.
Dr. Lipton’s team developed a new imaging method that allowed them to detect these subtle brain changes more precisely. Their scans showed that excessive heading made the boundary between gray and white matter appear "blurred," indicating possible tissue disruption.
“These subtle changes can impair brain function over time,” said Dr. Lipton.
As part of the study, participants underwent memory and learning assessments. Those who headed the ball more frequently performed slightly worse on average.
While the difference wasn't large, it was statistically significant, pointing to potential early signs of cognitive decline.
One of the most striking aspects of the study is that the findings apply not just to professional athletes or those with diagnosed head injuries, but to seemingly healthy amateur players as well.
Dr. Lipton emphasized, “This is the first study to clearly demonstrate that repeated head impacts in football can lead to specific structural changes in the brain, which may ultimately affect intellectual performance.”
Previous studies had noted changes in white matter of footballers’ brains compared to swimmers, but failed to conclusively link those changes with cognitive function.
This latest research identifies damage in a part of the brain—ZWI—that had largely been overlooked in earlier investigations, which may explain inconsistencies in prior results.
Researchers also warned that prolonged damage to this brain region could potentially lead to serious neurological conditions in the future, such as chronic traumatic encephalopathy (CTE); a degenerative brain disease commonly linked to repeated head trauma in sports.
The study has been published in the peer-reviewed journal JAMA Network Open and adds to a growing body of evidence calling for stricter safety guidelines and greater awareness about the risks of repeated heading in football.
