Premature menopause before 40: Causes and treatment

Menopause usually occurs between the ages of 45 and 55, but when it happens before 40, it is known as premature menopause or premature ovarian insufficiency.

It occurs when the ovaries stop producing eggs, leading to a drop in estrogen levels, and is confirmed after 12 consecutive months without a period. The transitional phase with symptoms like hot flashes before full cessation is called perimenopause, which can occur due to ovarian damage or natural early depletion.

Symptoms of Early Menopause

Irregular periods, unusually long or short menstrual cycles, heavy bleeding, spotting, and intervals longer than a week between cycles can signal premature menopause. Other common symptoms include mood swings, changes in sexual desire, vaginal dryness, sleep disturbances, hot flashes, night sweats, and loss of bladder control.

Common Causes

Genetic Factors: Family history plays a strong role; women whose mothers or close female relatives experienced early menopause are at higher risk.

Lifestyle Factors: Smoking affects estrogen levels and can cause menopause 1 to 2 years earlier. Vegetarian diets, lack of exercise, and insufficient sunlight exposure are also linked to earlier onset.

Chromosomal or Genetic Conditions: Conditions like Turner syndrome, where a female is born with an incomplete chromosome, often lead to premature ovarian failure.

Autoimmune Diseases: Diseases such as thyroid disorders or rheumatoid arthritis can cause the immune system to attack ovarian tissues, leading to inflammation and cessation of function.

Epilepsy: Women with epilepsy face a higher risk of premature ovarian insufficiency.

Diagnosis and Treatment

Diagnosis is generally based on physical symptoms, though medical consultation is advised to rule out other health conditions. Premature menopause usually requires treatment to replace hormones the body would naturally produce until the average age of menopause. Hormone Replacement Therapy (HRT) is the most common approach, though it carries potential risks of heart disease, stroke, and breast cancer.
